    Self Attestation

    The health and safety of our conference attendees is our top priority. Please view our Safety Policy and COVID Protocols below and confirm your acceptance by completing the self attestation form at the end.

    Safety Policy:

    For the safety of all attendees, HJ Sims will require that all conference attendees, including guests who plan to attend any HJ Sims-sponsored events, be fully vaccinated against COVID-19 as a condition to attend the HJ Sims 2022 Late Winter Conference OR provide proof of a negative COVID-19 test dated within 72 hours of arrival. Individuals are considered fully vaccinated two weeks after their second dose in a two-dose series such as the Pfizer or Moderna vaccines or two weeks after a single-dose vaccine such as Johnson & Johnson. All attendees must provide proof of vaccination to HJ Sims prior to attendance by completing the below Self Attestation. You will not be asked to show your vaccination cards.

    HJ Sims is committed to the health and safety for all of our conference attendees. As our top priority, we will follow the health and safety guidelines as identified by the host venue, Loews Portofino Bay Hotel on property at Universal Orlando.

    Face Coverings:

    Effective Friday, Dec. 24, 2021, face coverings are required at all public indoor locations within Universal Orlando Resort for both guests and employees regardless of vaccination status — including restaurants, shops, and indoor hotel public areas. Face coverings are also required at all attractions from the moment guests enter the queue to when they exit the experience. Guests are expected to bring a face covering with them for their visit.

    Approach to Lobbies & Guest Areas:

    • High alcohol content hand sanitizer stations have been added to our meeting space, front desks, restaurants, fitness centers, and other high traffic guest areas, as well as Team Member areas.
    • Lobby attendants will clean the elevator area frequently.
    • All event rooms’ tables, chairs and equipment will be regularly cleaned and disinfected.

    In order to ensure a safe experience for all attendees, we ask that you complete the following in advance of attending the HJ Sims 2022 Late Winter Conference.
